Submit a quote for the Mr Monkey t-shirt project

I (misha) am still looking for a new screen printer and/or tshirt supplier for this and possibly future projects. The main three things i'm looking for are:

  • Someone located in the Chicago area
    I'm trying to save money here, so i don't want to be paying to have stuff shipped to me. Plus, if i'm buying the tshirts from you, i need to be able to see the color and feel the fabric before i can decide what to go with.
  • I would prefer to deal directly with shop where the printing will be done
    In my personal experience, middlemen don't know enough about what's going on to be able to answer all my probing questions, nor even, it seems, to give me an accurate quote on the cost.
  • A printer who is capabale of using high quality vector images (like ai or eps)

I have a t-shirt project planned and am shopping around for quotes from screen printers; i have a lot of questions because i'm hoping to find someone to do all my t-shirts from now on. First, a brief description of this project:

I'll probably order a case of shirts, dark in color (black, slate blue, or brick). The design is 2 colors (gray and glow-in-the-dark white), it will be roughly 7" wide by 4" high, and i will most likely want it on the front of the shirt.

Based on this information, can you give me an estimate of the cost? I'm interested in knowing about the screen fees, setup fees, and the cost per shirt for the printing itself. If you think you'll need to do a "flash print," please be sure to include the cost of that screen in your estimate.


Additional questions:

How much more would it cost to add a small (2") 1-color (glow-in-the-dark white) design on the left sleeve?
How much more would it cost to add a small (2") 1-color (glow-in-the-dark white) design on the front (with the larger 2-color design on the back)?
If you sell blank t-shirts (i'm looking for 100% cotton shirts, and i probably want the heavier weight), what's the approximate cost per piece if i buy a case?
How about if i buy less than a case, like a dozen or so?
If you sell blank shirts, do you have samples available so i can see the color and feel the material before deciding which ones to go with? [yes]   [no]
